Feedback for Your Midwife

Feedback is important for every midwife, so they can be appreciated for their strengths and improve on their weaknesses. If you have concerns about your midwife, speaking to them is the first port of call, but if you cannot or feel your words were not heard or acted on, there are options for complaints processes on this page.

This feedback form from the NZ College of Midwives can be used for either your LMC or any hospital midwives that care for you. You can fill out feedback for as many midwives as you like – one form each.

If you have concerns or complaints about your midwife, the NZ College of Midwives has a Resolution Support Service here, and this is a good route to take if you feel you cannot speak to the midwife about your concerns.
